Reclast Checklist

For Reclast referrals, please make sure to follow this checklist and referral form listed HERE

Prior to contacting Kimberly @ IV Services, complete labs:

  1. Calculated Creatinine Clearance > 35 mL/min.
  2. Serum calcium is within normal levels
  • Please Fax Reclast Referral Form to Kimberly at (504)392-0973 or email to ivservices2004@aol.com.
  • Include a copy of the prescription, front and back copies of the patients insurance card, and copies of the lab work.
  • IV Services will pre-certify the patient’s insurance and contact the patient with appointment time and out of pocket cost for the patient.
  • Please give IV Services a couple of weeks to gather all this information.
  • IV Services will also fax a document to your office with patients date of infusion for your records.

Remember:

  • If patient is already taking an oral bisphosphonate therapy make sure Reclast is administered at the next scheduled dose, either daily, weekly or monthly depending on the medication. If the patient is getting Boniva injections infuse Reclast on the date of the next scheduled Boniva injection.
  • Make sure patients are aware to be adequately hydrated prior to infusion.
  • Recommend that all women with osteoporosis take 1,200mg of calcium and 800 IU of Vitamin D daily.
  • Instruct patients on the potential flu-like symptoms, fever, muscle or joint pain and headache. Most occur in the days following treatment. A mild pain reliever such as acetaminophen may reduce these symptoms. After the second treatment, these side sffects were much less common.
